JOB DUTIES AND RESPONSIBILITIES This independent professional public health nursing position is located at the Bithlo Neighborhood Center for Families. The incumbent provides professional nursing services to identify and qualify participants in programs such as schools, homes, clinics, and community settings. Nursing support is provided at outreach/awareness and incident response events within the community, including mass immunization, emergency response activities, and community health engagements. Support may be required outside normal business hours, including evenings and weekends. The Senior Community Health Nurse will provide case management services at NCF locations (Taft, Union Park, Bithlo/Christmas, Englewood, and Pine Hills). Case management includes identification of needs/goals; assessments; counseling (including education); ensuring participants can access medical care; referrals and follow‑up; and holistic care planning. The nurse conducts basic health screenings, immunization case management, health education, fluoride treatment program, family counseling, and management of students’ medical conditions. Services are provided through individual contacts and conferences with teachers, students, and parents for opt‑in students. Referral requests from school staff lead to home or school visitation assessments and subsequent referrals. Immunization services are delivered at NCF locations during peak demand periods, following CDC schedule guidelines. Vaccine storage, handling, and accountability are maintained, as well as monitoring vaccine temperature logs, uploading data into Florida Shots, and managing inventory. The nurse focuses on health issues in assigned schools for opt‑in students, including vision, hearing, growth and development, and scoliosis. Home visits are performed as needed. Health education lectures, audiovisual aids, individual contacts, and conferences promote school and community health. Senior citizens receive health screenings, education, and counseling for chronic diseases such as diabetes and hypertension. The nurse coordinates community health activities such as health fairs, screenings, wellness promotion, youth development, and immunizations. The nurse reviews quarterly NCF reports, supports staff, trains new nurses, ensures EAR entry, attends NCF meetings, CCC Conference, and OCHD meetings, and maintains client records and confidentiality. Other duties as assigned are performed as required. MINIMUM QUALIFICATIONS Active licensed Registered Professional Nurse in accordance with F.S. 464 or eligible to practice nursing in accordance with F.A.C. 210-6.27 and have one year of professional nursing experience. A minimum of 6‑month immunization experience. Willingness to work during or beyond normal work hours or days during emergencies, including Red Cross shelters or disaster responses. Ability to perform basic nursing skills during emergency duties, including vital signs, health assessment, safety, hygiene care, wound care, and infection control. Willingness to work at NCF Englewood or NCF Bithlo locations and required to work 6 Saturdays per year. Daily in‑office or field work required. KNOWLEDGE, SKILLS AND ABILITIES, INCLUDING UTILIZATION OF EQUIPMENT, REQUIRED FOR THE POSITION Ability to administer vaccines across all age groups. Comprehensive knowledge of vaccines, immunization requirements, and guidelines for infants, children, adolescents, and adults. Proficiency in FLSHOTS and HMS systems for accurate documentation and reporting. Strong communication skills, fluency in spoken and written English; bilingual proficiency in a second language preferred. Knowledge of childhood and adult immunization schedules, including state and federal guidelines. Understanding of Florida Department of Education school immunization requirements to ensure compliance with mandated standards for student enrollment. Ability to learn training materials and apply knowledge effectively to job duties. Ability to teach others, sharing knowledge and skills in a clear and supportive manner. Knowledge of adult learning methods to enhance training and education effectiveness. Ability to sit for extended periods while remaining focused and attentive. Ability to work independently, demonstrating initiative and accountability. Proficiency in using multiple computer programs to support clinic operations and documentation. Availability to attend evening and weekend immunization clinics and events as needed. Flexibility to adjust work schedule and responsibilities.
