Description

• Assess patients within inpatient or outpatient settings and create personalized treatment plans
• Implement physical therapy interventions aimed at enhancing mobility, strength, balance, and overall function
• Work closely with nurses, physicians, case managers, and other rehabilitation professionals to ensure optimal patient outcomes
• Educate patients and their families on therapeutic objectives, discharge planning, and at-home exercises
• Maintain thorough documentation of evaluations, treatments, and patient progress following facility and regulatory standards
• Engage in care team rounds and interdisciplinary discussions to ensure coordinated and comprehensive care

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution

Certifications

• State licensure as a Physical Therapist
• CPR certification highly regarded
