(RN) Registered Nurse - Home Health | Vinton, Ohio

Advance your nursing career with a vital role caring for patients in their homes. This position offers a full-time, 13-week contract for an experienced RN skilled in Home Health. Youll work Monday through Friday, 8:00 AM 5:00 PM, managing a caseload that typically involves 35 patient visits per day (30 points per week). The role is entirely field-based, providing essential care and building strong, impactful relationships with those you serve.

Key Responsibilities:

• Conduct thorough OASIS assessments and develop tailored care plans
• Deliver skilled nursing interventions, including wound care, IV therapy, medication administration, and chronic disease management
• Monitor patient status, report changes, and communicate proactively with healthcare teams and physicians
• Educate patients and caregivers about diagnoses, therapy plans, medications, and preventive health practices
• Ensure precise and timely documentation using Home Care Home Base (HCHB)
• Work collaboratively with physical therapists, occupational therapists, social workers, and home health aides for comprehensive care coordination
• Attend case conferences, team meetings, and participate in ongoing education as needed
• Adhere to agency and regulatory standards for quality and compliance

Qualifications:

• Active and unrestricted RN license in the state of practice
• At least 1 year of recent home health nursing experience, with field work preferred
• Proficient in Home Care Home Base (HCHB) and experienced with OASIS
• Reliable transportation with valid drivers license and auto insurance
