Clinical Team Manager

Salary:
$110,000-$130,000 (Negotiable, depending on experience) plus health benefits, PTO, 401k with match, and more
 
Job Summary:
Our client is seeking a Clinical Team Manager to lead a remote nursing team delivering chronic care management services. This role is responsible for overseeing day-to-day clinical operations, developing team performance, and building scalable systems to support a growing patient population. This is a unique opportunity to step into a leadership role within a fast-growing healthcare organization focused on transforming chronic care delivery.
 
Job Responsibilities:

• Lead and manage a remote team of nurses across multiple time zones
• Develop and implement KPIs, workflows, and SOPs
• Hire, onboard, and train clinical staff
• Ensure quality and consistency across patient interactions
• Maintain compliance with clinical and regulatory standards
• Mentor and develop team members to drive performance
• Support operational scaling and process improvement initiatives

 
Qualifications:

• Active RN license (Multistate)
• Prior experience managing clinical or nursing teams
• Experience in CCM, PCM, or chronic care management
• Strong leadership, communication, and organizational skills
• Experience working in remote or telehealth environments is a plus
• Ability to thrive in a fast-paced, evolving environment
