Responsibilities
- Review medical records to assess utilization and determine medical necessity and appropriate level of care.
- Coordinate with the interdisciplinary team to support discharge planning and transitions of care.
- Verify payer requirements and regulatory standards, documenting decisions in the electronic health record.
- Communicate care plans and decisions with treating clinicians, patients, and families as appropriate.
- Monitor gaps in care, participate in case management activities, and support quality and performance improvement initiatives.

Requirements
- Active Registered Nurse license in Pennsylvania (or ability to obtain PA licensure)
- Knowledge of utilization management principles, medical necessity criteria, and payer guidelines
- Experience in a long-term care setting or geriatrics is helpful
- Proficiency with electronic health records and medical documentation
- Strong communication and teamwork skills; ability to work in a fast-paced, collaborative environment
