Care Coordinator

Salary: $44,000 annually About the Role Pathways is seeking a compassionate, organized, and detail-oriented Care Coordinator to support individuals receiving services through our Certified Community Behavioral Health Clinic (CCBHC). The Care Coordinator plays a vital role in ensuring consumers receive seamless, integrated care by connecting them with behavioral health, physical health, and community-based resources that promote wellness, recovery, and overall quality of life. This position works collaboratively with consumers, providers, and community partners to coordinate services and support a person-centered plan of care. Education Requirements Minimum Associate Degree or Diploma in Nursing required Current Kentucky nursing license required Experience Requirements Experience working with and accessing community resources Proficiency in accurate and timely documentation within an electronic health record (EHR) Demonstrated ability to collaborate effectively with multidisciplinary teams Experience working with special populations served under CCBHC guidelines preferred Strong organizational skills and ability to manage multiple priorities simultaneously Experience in public health, care coordination, social services, or behavioral health settings preferred Essential Duties and Responsibilities Coordinate services for consumers across physical health, behavioral health, and community support systems Facilitate linkages to community resources, including: Social services Housing assistance Educational programs Employment services Healthcare providers Other support services as needed Ensure smooth transitions of care between providers and levels of service Develop and maintain collaborative relationships with community partners and service providers Support consumers in achieving goals identified in their person-centered plan of care Document all care coordination activities accurately and timely within the electronic health record Monitor consumer progress and assist in addressing barriers to treatment and recovery Participate in multidisciplinary treatment team meetings and case consultations Maintain compliance with CCBHC standards, agency policies, and regulatory requirements Advocate for consumer needs while promoting independence, wellness, and recovery Knowledge, Skills, and Abilities Strong interpersonal and communication skills Ability to build rapport with consumers from diverse backgrounds Knowledge of community resources and referral processes Excellent organizational and time management skills Ability to maintain confidentiality and professional boundaries Proficiency with electronic health records and Microsoft Office applications Commitment to person-centered, trauma-informed, and recovery-oriented care This is a full time position. #J-18808-Ljbffr
