Thisis an excitingopportunity to makea meaningful impactby coordinating carefor patients withchronic and complexconditions while partneringclosely with physicians, advanced practice providers, and amultidisciplinary careteam. As an AmbulatoryRN, you willprovide clinical triage, patient assessment, education, care coordination, andongoing follow-up. You'll manage patientportal communications,conduct nurse visits, support treatment plans, and serveas a trustedresource for patientsthroughout their carejourney. SCHEDULE: Full-Time Monday–Friday |8:00 AM–4:30 PM QUALIFICATIONS: Current Massachusetts RegisteredNurse (RN) license. Current AHA BLS certification. Minimum of two(2) years ofRN experience. Strong assessment, triage, patient education, andcare coordination skills. Excellentverbal, written, andinterpersonal communicationskills. Demonstrated ability tocollaborate effectivelywith providers andinterdisciplinary teams. Commitment to deliveringcompassionate, patient-centered care toindividuals from diversebackgrounds. Ambulatory or specialtypractice nursing experience. Experience caring forpatients with chronicor complex medicalconditions.
