NOW HIRING: PHYSICAL THERAPIST --OUTPATIENT

Are you a Physical Therapist who enjoys helping patients regain strength, mobility, and independence?

Harvest Healthcare Services is looking for a dedicated  Physical Therapist to join our  Outpatient Therapy team. This position provides an opportunity to work directly with patients to develop individualized treatment plans, achieve functional goals, and support their return to the activities that matter most to them.

Our Physical Therapists work collaboratively with patients, families, Physical Therapist Assistants, healthcare professionals, and other members of the rehabilitation team to provide high-quality, patient-centered care.

What You’ll Do

• Evaluate patients to identify physical therapy needs and develop individualized treatment plans.
• Conduct screenings to determine the need for physical therapy intervention and treatment.
• Perform comprehensive and specialized evaluations as indicated.
• Establish appropriate short- and long-term goals based on each patient’s condition, abilities, and desired outcomes.
• Develop and implement treatment plans designed to improve  strength, balance, mobility, flexibility, coordination, endurance, and functional independence .
• Provide physical therapy treatments in accordance with established standards of practice, company procedures, and therapy department procedures.
• Monitor each patient’s response to treatment and modify interventions as appropriate to help achieve established goals.
• Develop recommendations, strategies, and programs to help patients maintain and improve their functional abilities outside of therapy.
• Educate patients and families on physical therapy procedures, exercises, strategies, recommendations, and home programs to encourage continued progress outside of therapy sessions.
• Recommend and provide adaptive equipment or other assistive devices as appropriate and educate patients and families on their safe use.
• Communicate evaluation findings, treatment plans, recommendations, and progress with patients, families, physicians, and other healthcare professionals.
• Coordinate patient therapy schedules with other members of the rehabilitation team.
• Collaborate with other healthcare professionals to support a comprehensive approach to patient care.
• Participate in discharge planning and assist patients in transitioning to independent management of their condition.
• Supervise and direct Physical Therapist Assistants in accordance with applicable regulations and established plans of care.
• Supervise Rehabilitation Aides, Physical Therapy Aides, students, and volunteers as directed.
• Provide education to the rehabilitation team, patients, families, and community as appropriate.
• Maintain accurate and timely documentation and billing of physical therapy services.
• Participate in quality assurance activities and department initiatives.
• Maintain productivity and efficiency while providing quality patient care.
• Attend and participate in required training, educational programs, meetings, and professional development opportunities.
• Maintain a clean, professional, and safe outpatient therapy environment.
• Follow HIPAA, confidentiality, infection control, universal precautions, OSHA, and other applicable healthcare standards.
• Report hazardous conditions, incidents, injuries, and equipment concerns promptly.
• Respond appropriately to emergency situations.
• Adapt positively to changes in responsibilities, patient needs, and department priorities.
• Communicate professionally and respectfully with patients, families, coworkers, physicians, and other healthcare professionals.
• Perform other duties as assigned.

What We’re Looking For

• Minimum of a  Bachelor of Science Degree in Physical Therapy from an accredited program.
• Passing score on the  National Physical Therapy Examination (NPTE) .
• Current licensure as a Physical Therapist in the state(s) of practice.
• Current certification/registration as required.
• Commitment to ongoing continuing education and professional development.
• Strong evaluation, clinical reasoning, treatment planning, and communication skills.
• Ability to develop individualized treatment plans and establish meaningful patient goals.
• Ability to work effectively with patients of varying abilities and rehabilitation needs.
• Strong organizational and time-management skills.
• Ability to work independently while collaborating effectively with a multidisciplinary team.
• Professional, dependable, flexible, and patient-centered approach.
• Commitment to maintaining patient privacy, dignity, and confidentiality.

Physical Requirements

• Ability to operate a computer and other office equipment.
• Ability to sit and work at a computer for extended periods.
• Ability to lift up to 50 pounds .
• Ability to walk throughout the outpatient therapy department.
• Ability to tolerate frequent or repetitive standing.
• Ability to traverse stairs.
• Ability to stoop, bend, squat, kneel, and reach as required when performing daily responsibilities.
• Ability to safely assist patients during therapeutic activities and functional mobility.
• Ability to maintain predictable attendance.
