The Community Outreach Coordinator is responsible for expanding access to services for pregnant and parenting women affected by substance use disorders, with a particular focus on underserved and rural communities. This position serves as the primary liaison between the program and community partners, ensuring timely outreach, referrals, admissions, priority admissions, waiting list management, and capacity notifications. The Community Outreach Coordinator works collaboratively with healthcare providers, behavioral health organizations, social service agencies, child welfare partners, recovery support services, and community stakeholders to increase awareness of program services and reduce barriers to treatment and recovery support. The position plays a critical role in ensuring equitable access to care and maintaining compliance with referral and admission policies.

• Develop and implement outreach strategies to increase awareness of Family Ark program services among pregnant and parenting women affected by substance use disorders. 

• Establish and maintain collaborative relationships with referral sources, including hospitals and healthcare systems, OB/GYN practices, primary care providers, behavioral health and substance use treatment providers, child welfare agencies, public health departments, community-based organizations, recovery community organizations and community corrections. 

• Conduct presentations, educational sessions, and community engagement activities to promote program services. 

• Represent the program at community meetings, coalitions, conferences, and networking events. 

• Develop targeted outreach initiatives for rural communities and populations experiencing barriers to care. 

• Collaborate with program leadership and Development Office to develop innovative approaches for reaching underserved populations. 

• Maintain accurate records of outreach activities, referrals, admissions, waiting lists, and community partnerships.

• Track outreach and access metrics to evaluate program effectiveness.

• Prepare monthly, quarterly, and annual reports as required by leadership, grant and other funding agencies.

• Utilize electronic data systems, including ER, to document activities and outcomes. 

• Seeks feedback from referral sources related to Family Ark provision of services, compiles and reports data.

• Implements quarterly strategic referral management action plans that support agency
  census and QI goals as outlined by the Chief Operating Officer. Provides consistent updates to the VP of Programs regarding census. 

• Accurately communicates all Family Ark continuum of services to referral sources and communities. 

• Initiates and promotes customer service, problem solving and ensuring quality improvement. 

• Maintains a working knowledge of community resources, current trends and best practices in child welfare.

• Promotes cohesion with all communications, internally and externally. 

• Other duties and responsibilities as assigned. 

Minimum Requirements  

• Bachelor’s Degree in social work, human services, psychology, public health or related field preferred. 

• Minimum of two years of experience in community outreach, care coordination, behavioral health, social services, or a related field. 

• Marketing and/or sales experience preferred. Experience implementing new programs a plus. 

• Knowledge of substance use disorders, recovery supports, and maternal health issues. 

• Ability to interact effectively with all people involved with Family Ark. 

• Strong verbal and written communication skills.

• Basic computer and word processing skills. 

• Skilled at negotiating with diverse key stakeholders while maintaining professionalism. 

• Demonstrates creative thinking, and a positive, can-do attitude. 

• Must be able to maintain confidentiality of Family Ark information and professional boundaries. 

• Ability to work independently, under pressure, to meet deadlines. 

• Willingness to travel. 

• Ability to interact effectively with individuals with diverse social and economic backgrounds.

• Must demonstrate a commitment to quality performance and service excellence.
