Registered Nurse Rehabilitation

Job Summary:


We seek a highly skilled and experienced Registered Nurse specializing in Rehabilitation to provide skilled nursing care, case management, and patient support in a rehab setting. The Registered Nurse will perform patient assessments, medication administration, treatment planning, and collaborate closely with the interdisciplinary team to ensure optimal patient outcomes. This role requires strong communication, collaboration skills, and a minimum of two years of nursing experience.

Responsibilities:

• Perform comprehensive patient assessments and develop individualized care plans.
• Administer medications and treatments as prescribed.
• Collaborate with interdisciplinary teams including physicians, therapists, and other healthcare providers.
• Provide skilled nursing care to patients in a rehabilitation environment.
• Manage patient documentation and maintain accurate medical records.
• Assist in patient education and discharge planning.
• Float between Medical/Surgical and Orthopedic units as needed.
• Participate in on-call duties depending on patient census.

Required Skills & Certifications:

• Active Registered Nurse license in the state of California.
• Minimum of 2 years nursing experience, preferably in hospital, home health, or hospice care.
• Bachelor's degree in Nursing or related field.
• Clinical assessment, medication administration, and treatment planning proficiency.
• Strong interdisciplinary team collaboration skills.
• Excellent communication and interpersonal skills.
• Driver's license, car insurance, and reliable automobile for field work.

Preferred Skills & Certifications:

• Experience in Medical/Surgical, ICU, ER, Psychiatric, Labor & Delivery, PACU, or Operating Room settings.
• Experience working with rehabilitation patients and related therapies.

Special Considerations:

• Weekend work required every other weekend.
• Call duties required depending on patient census.
• Float assignments possible between Medical/Surgical and Orthopedic units.
• Return to Office (RTO) restrictions limit to no more than 7 days.
• Work environment includes a 7-room, 14-bed facility with an average daily census of 8 patients.

Scheduling:

• Start date is ASAP.
• Shift patterns include weekends every other weekend and possible float shifts.
• Schedules determined based on patient census and staffing needs.
