HIV Care Case Manager I

CAN Community Health in Miami is hiring a Medical Case Manager I to coordinate comprehensive case management for clients affected by HIV, Hepatitis C, STI's and other infectious diseases. The position offers a full-time day shift with 8:00 am–5:00 pm M-Thu and 8:00 am–12:00 pm Fri, and pay $21-$24 per hour depending on experience. You will screen eligibility, develop care plans, link to services, coordinate with healthcare teams, document encounters within 48-72 hours, and support patients #J-18808-Ljbffr CAN Community Health
