Description: The RN implements professional nursing practice centered on the patient as a whole person. The focus is on the creation of a caring environment aimed at healing and wellness. Nursing practices are based on evidence, research, and hospital policies and procedures. Clinical decision-making incorporates all four phases of the nursing process, critical thinking, and creativity. Nursing excellence is promoted through collaborative relationships among nursing staff and across other disciplines. Responsibilities include coordinating, ensuring, and providing direct patient care activities on the unit. This involves performing nursing rounds, assessments, and treatments. Facilitating admission, transfer, and discharge activities is also key. Education for patients and families is provided, alongside care coordination and treatment planning. Registered nurses must perform and document comprehensive assessments according to policies and procedures. Consideration of the patient’s culture, age, and developmental level is crucial. Identifying patient risk factors and collecting health information from family or others may be necessary. Objective and subjective data should be collected thoroughly. A problem list should be developed, prioritizing needs based on input from the inter-professional team. Action plans and interventions should be relevant to the patient's cultural and age considerations. Desired outcomes are determined in coordination with patients and families. Patient care therapies are implemented within the established scope of practice and policies. Procedures, treatments, and processes must be explained to patients and families. Initiatives to alleviate stress and discomfort for patients are essential. Changes in patient condition must be recognized and reported appropriately. Ongoing assessments and responses to therapies must be documented in accordance with standards. Tasks may be delegated and oversight of team activities regarding patient care is necessary. The accuracy and completeness of delegated patient care should be monitored. Educational needs of patients related to prioritized problems should be identified and addressed. Facilitating the patient discharge process in a timely manner is important. Input into patient/nurse assignments considering staffing and patient acuity is also a duty. Active participation in unit-based activities that support organizational values and priorities is expected. Promoting a positive group dynamic and healing environment is crucial. Assuring nursing excellence in accordance with accepted standards and best practices is a priority. Participation in department quality improvement activities is required.
