Field Care Navigator | Patient Advocate & Care Planner

UnitedHealthcare is seeking a Field Care Coordinator to assess and plan for enrollees, traveling to homes, facilities, and ALFs to perform comprehensive assessments and develop care plans. You will collaborate with the interdisciplinary team to deliver coordinated, cost-effective care.
