Schedule:

• Flexible shifts offered between Monday and Sunday
• Self-scheduling; design your own calendar
• Committed minimum hours weekly or bi-weekly

Responsibilities:

• Complete comprehensive annual and post-discharge home health risk assessments (each visit approximately 45 minutes)
• Review and document prior/current medical histories, medical conditions, and medication regimens
• Provide clear, practical education to members on their health and prescribed medications
• Identify gaps in care, offering actionable recommendations for improved health outcomes
• No requirement for the development of treatment plans or prescribing medications

Qualifications:

• Active Nurse Practitioner license in North Carolina
• MSN degree from an accredited program
• Board Certification by AANP or ANCC
• Current BLS-CPR certification
• At least 6 months of Nurse Practitioner experience preferred
