Care Navigator (SW), Variable Part-Time, Inpatient Rehabilitation

Care Navigator – Inpatient Rehabilitation Adventist Rehabilitation – Rockville We are seeking an experienced Care Navigator for our Inpatient Rehabilitation department who will embrace our mission to extend God’s care through the ministry of physical, mental, and spiritual healing. Responsibilities Complete comprehensive psychosocial assessment focusing on patient’s requirements for transition to the next level of care. Collaborate with physicians, nurses and other disciplines involved in the patient’s care to foster a coordinated discharge planning approach. Identify and navigate patient testing and treatment to reduce barriers to discharge and prevent delays; communicate barriers to leadership for resolution and trending. Communicate with Utilization Review staff on any denials, issues or barriers to discharge. Identify community services and resources and assist with patient connection to these services. Participate in interdisciplinary rounds and other patient care conferences. Document assessments and interventions according to departmental standards. Participate in process improvement activities. Qualifications Bachelor of Social Work (BSW) or Master of Social Work (MSW). Minimum of 3 years of experience in a diverse clinical setting. Prior case management or discharge planning experience preferred. Prior experience with Cerner EMR preferred. Active LBSW, LMSW or LCSW licensure. Active American Heart Association Basic Life Support (BLS) certification required. Work Schedule Employment Type: Variable Part‑Time. Typical Daily Schedule: As needed for PTO coverage. Shift Type: Day. Weekend Requirements: Occasional. Holiday Requirements: Occasional. On‑Call: Occasional. Shift Differential Eligible: No. Pay Pay Range: $34.58 – $51.87 per hour.
