Join Mountain View Health & Rehabilitation as a Clinical Liaison and help patients transition from the hospital to post-acute care while building strong relationships with hospitals, physicians, and referral partners throughout the Carson City area.

Responsibilities

Build relationships with hospitals, case managers, physicians, and referral partners
Evaluate referrals and coordinate admissions
Assist with clinical screenings and pre-admission assessments
Partner with facility leadership to grow admissions and improve patient outcomes

Qualifications

Nevada LPN or RN license preferred
Experience in skilled nursing, post-acute care, case management, discharge planning, admissions, or healthcare marketing preferred
Strong communication and relationship-building skills
Local travel required
