Field Social Worker - Hospice

Day (United States of America) Field Social Worker – Hospice Responsibilities Conduct psychosocial assessments and participate in developing interdisciplinary care plans for hospice patients and families. Provide psychosocial support to patients and caregivers. Document psychosocial assessments. Facilitate community referrals. Facilitate nursing home/assisted living facility transfers. Assess bereavement needs. Provide crisis intervention. Serve as a resource to the interdisciplinary team. Participate in community education regarding hospice care. Qualifications MSW from a Council on Social Work Education (CSWE) accredited school. Valid driving license and good driving record. At least one year of healthcare experience. At least one year of social work experience in a health care setting. Background in social work or counseling and knowledge of community resources. Working knowledge of systems theory and family dynamics. Strong clinical and assessment skills, including crisis intervention and the ability to work in a team setting. Excellent organization, time‑management, and self‑awareness. Customer‑service oriented with strong verbal and written communication skills. Professionalism in interpersonal communication with colleagues, physicians, and ancillary department personnel.
