Nurse Practitioner

Job Summary:

We are seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) with a background in Geriatrics or Internal Medicine to deliver high-quality preventative care to residents in skilled nursing facilities. As a Nurse Practitioner at Carease Health - FL, you will have the opportunity to work in a hybrid role, combining on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation.

About the Role:

As a key member of our clinical team, you will be responsible for:

• Building strong relationships with facility staff and maintaining a patient-centered approach

• Delivering high-quality preventative care to residents in skilled nursing facilities

• Collaborating with our experienced clinical team to ensure seamless alignment with facility processes and protocols

• Conducting on-site visits and documenting patient care remotely with the support of our scribe team

Requirements:

• Active NP or PA license (in the state of practice)

• 1+ year of experience in:

  • Geriatric care

  • Documentation review

• Excellent communication, collaboration, and teamwork skills
