Develop, implement, and regularly reassess individualized, patient-centered plans of care in collaboration with patients and families. Deliver evidence-based interventions across complex diagnoses, including neurological, oncological, orthopedic, cardiopulmonary, and medically complex conditions. Collaborate with the interdisciplinary team to coordinate care, optimize outcomes, and support safe, effective discharge planning. Schedule will have 2 evenings a week until approximately 7:15 p.m. The other days are 7 a.m. to 3:30 p.m. What Minimum Requirements You’ll Need Licensure / Certification / Registration: BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted. Physical Therapy (PT) credentialed from the Illinois Department of Financial and Professional Regulation prior to hire date or job transfer date required. A valid license pending letter will be accepted until license is issued. Education Bachelor's degree required. Master's OR Doctorate degree preferred. What Additional Preferences We're Seeking Prior Ortho outpatient experience.
