Charleston, WV 25387 Lewisburg office 1265 Maplewood Ave. Lewisburg, WV 24901 RN Case Manager Job Location: Charleston, WV and surrounding areas Position Summary: Provides skilled nursing care, case management, and other clinical support services to patient/families in the facility/home setting.Performs other job duties as assigned. Qualifications: Graduate of an approved nursing program. Strong organizational, problem-solving, and people skills. Contingent upon successful completion of drug screen, background check, and other screenings as applicable. Exposure to infectious diseases, bloodborne pathogens, and bodily fluids. Frequent interaction with patients, families, and interdisciplinary team members. Fast-paced clinical settings with potential for high stress. Required use of personal protective equipment (PPE). Physical Requirements: Ability to lift, transfer, and reposition patients up to 50 pounds independently and over 50 pounds with assistance. Frequent standing and walking for extended periods (8–12 hour per shift). Frequent bending, stooping, kneeling, crouching, and reaching. Ability to push and pull equipment (e.g., wheelchairs, stretchers, and medication carts). Visual acuity to read medication labels, physician orders, and monitors.
