Job Title: Occupational Therapist (OT) Profession: Occupational Therapist Specialty: Home Health Duration: 13 Weeks Shift: 8:00 AM - 4:30 PM Hours per Shift: 8 Experience: 2+ Years Current Occupational Therapy Experience, 2+ Years Current Home Health Experience, OASIS Experience License: Active Michigan License Certifications: Active American Heart Association BLS Must-Have: Traveler must have their own vehicle and a clean driving record. Proof of auto insurance is required. Traveler's permanent address must be included in the profile. All scheduling requests must be made at the time of submission. Description: No requests for time off will be accepted for this assignment. Candidates living within 50 miles of the facility will not be considered. Typical daily mileage is approximately 35-50 miles. Mileage will be paid at the current Federal rate, calculated to and from the main office only. Dress code is olive green scrubs. Non-billable orientation is 8 hours. Holiday and overtime multipliers are 1.25%. A skills checklist, current to 1 year, pertinent to the specialty of Occupational Therapy is required. A skills checklist, current to 1 year, pertinent to the specialty of Home Health is required. Two supervisory reference forms completed within the last two years are required.
