Job Description

• NO RTO WILL BE ACCEPTED FOR THIS ASSIGNMENT Client will not consider candidates who live within 50 miles of facility Typical Daily Mileage: approx 35-50 miles Mileage paid at current Federal Rate
• To be calculated TO and FROM Main office ONLY (not from housing) Dress Code: Olive Green Scrubs Non-billable Orientation: 8hrs Holiday and OT Multipliers: 1.25%

• 2+ Years current OT experience Experience [REQUIRED]
• 2+ Years current HOME HEALTH experience Experience [REQUIRED]
• OASIS experience [REQUIRED]
• Traveler must have their own vehicle for this need/Clean driving record
• Proof of auto insurance [REQUIRED]
• Traveler's perm address must be included in profile
• Any/All RTO must be included in profile. ALL SCHEDULING REQUESTS MUST BE MADE AT THE TIME OF SUBMISSION. Facility will NOT approve scheduling requests at/after time of offer/at interview. Please obtain this information prior to submittal. Requests after offer may lead to rescinded offer
• Work history
• Skills Checklist, current to 1 year, pertinent to the specialty Occupational Therapy [REQUIRED] [DOCUMENT]

• 2 Supervisory Reference Forms Completed within the last two years [REQUIRED] [DOCUMENT]
• Active American Heart Association BLS [REQUIRED] [DOCUMENT]
• LARA/Active Michigan/MI License [REQUIRED] [DOCUMENT]

Certification Requirements

• BLS (AHA)
