Occupational Therapist - Home Health
This is a 13-week contract position.
Shift: 8:00 AM - 4:30 PM, 8-hour days with a 40-hour guarantee.
The Occupational Therapist will provide therapy services to patients in their homes. Responsibilities include assessing patient needs, developing treatment plans, and conducting therapy interventions.
Responsibilities:
- Conduct patient assessments and evaluations.
- Develop and implement individualized treatment plans.
- Monitor patient progress and adjust treatment accordingly.
- Document patient care and maintain accurate records.
- Communicate with the healthcare team to coordinate patient care.
Required Experience / Certifications / Licensure:
- 2+ years of current OT experience.
- 2+ years of current home health experience.
- OASIS experience required.
- Active MI Occupational Therapy license.
- Current American Heart Association BLS certification.
- Must have a clean driving record and proof of auto insurance.
