Occupational Therapist - Home Health

Assignment Duration

This is a 13-week contract position.

Schedule

Shift: 8:00 AM - 4:30 PM, 8-hour days with a 40-hour guarantee.

Job Description

The Occupational Therapist will provide therapy services to patients in their homes. Responsibilities include assessing patient needs, developing treatment plans, and conducting therapy interventions.

Responsibilities

• Conduct patient assessments and evaluations.
• Develop and implement individualized treatment plans.
• Monitor patient progress and adjust treatment accordingly.
• Document patient care and maintain accurate records.
• Communicate with the healthcare team to coordinate patient care.

Required Experience / Certifications / Licensure

• 2+ years of current OT experience.
• 2+ years of current home health experience.
• OASIS experience required.
• Active MI Occupational Therapy license.
• Current American Heart Association BLS certification.
• Must have a clean driving record and proof of auto insurance.
