Heartline Staffing, in partnership with Purple Cow Recruiting, is hiring a Family Medicine Physician in Willow Creek, California.

POSITION OVERVIEW
• Outpatient-only Family Medicine role serving a mission-driven rural Northern California community
• Opportunity to care for a diverse patient population with complex primary care needs
• Collaborative care environment with integrated behavioral health and specialty support
• Strong team structure including medical assistants, RN support, and virtual triage services
• Flexible full-time scheduling with strong onboarding and mentorship support

• Flexible full-time outpatient schedule
• 5x8 shift structure
• Outpatient-only Family Medicine

• Quality improvement and interdisciplinary care participation

COMPENSATION & BENEFITS
• Annual compensation range: $234,635.20–$267,624.00
• $12,400 annual access bonus paid biweekly
• Up to $44,000 annual production bonus potential
• $50,000 sign-on plus retention bonuses for partnership-eligible providers
• Alternative retention structure for non-partnership track providers
• $10,000 relocation reimbursement
• Additional $10,000 relocation assistance for parent or in-law
• 29 days paid time off including CME, holidays, and sick time

• $5,000 annual company credit card for licensing, DEA, CE, and equipment
• Laptop and cell phone provided
• NHSC and federal loan repayment eligibility ranging from $75,000–$300,000

IDEAL BACKGROUND
• MD or DO with Board Certification or Eligibility in Family Medicine
• Active California medical license or ability to obtain
• Mission-driven mindset with interest in underserved rural healthcare
• Comfortable managing outpatient primary care panels
• Collaborative, team-oriented provider approach
• New graduates welcome; experienced physicians preferred
