Job Summary We are seeking a motivated Social Worker to provide high-quality social work services to our residents and their families. The successful candidate will possess excellent communication and interpersonal skills, with the ability to work effectively with a diverse population. As a member of our interdisciplinary team, you will be responsible for assessing social service needs, developing and implementing individualized plans of care, and facilitating connections with community resources. Responsibilities Conduct initial and ongoing assessments of each resident's social service needs Develop and implement individualized plans of care in collaboration with the interdisciplinary team Coordinate connections with community resources, including home healthcare agencies, hospices, and other social services Provide support and counseling to residents and their families, addressing concerns and needs related to social services Maintain accurate and up-to-date records of resident social service needs and interventions Participate in staff meetings and contribute to the development of policies and procedures related to social services Requirements Bachelor's degree in Social Work or related field Licensure as a Social Worker in the State of Pennsylvania 2 years of experience in a healthcare setting FULL_TIME position with competitive pay (salary to be determined) Benefits (TO BE DETERMINED) #J-18808-Ljbffr
