Care Coordinator - BBHE

Salary: $49,700/year An 4‑year federal grant‑funded position supporting Family Partnerships of Central Florida. The role centers on providing comprehensive family and child services, ensuring compliance, and enhancing the well‑being of children and families across the program. Responsibilities Provide family team confidence and support, ensuring effective planning and monitoring of family services. Deliver essential service‐administering functions, ensuring program delivery aligns with quality standards and regulatory requirements. Utilize resource management, coordinating community partnerships and staffing to meet client needs. Maintain relationships with families, caregivers and community providers, advocating for client rights and resources. Adhere to SOC values & principles, embodying ethical practice while advancing program effectiveness. Plan, schedule, and manage multiple deadlines, ensuring smooth operation of staff and services. Perform health‑care tasks such as medication delivery (where required), performing assessments, and supporting therapeutic activities. Qualifications Associate degree from an accredited college or 60+ college credits plus two years of full‑time social work or human services experience. Bachelor’s degree in a related field (social work, psychology, education, family development, etc.) preferred. Three or more years of experience working with families and children, including direct service provision. Strong ability to collaborate with multidisciplinary teams and to coordinate community resources. Demonstrated commitment to equity and cultural competence in service delivery. Experience Experience includes, but is not limited to, child advocacy roles, family support work, group home operations, educational support, nursing or therapeutic assistance, and other roles providing trauma‑oriented services. Prior experience with federal grant programs and knowledge of compliance requirements is advantageous. Education & Technical Skills Proficient in Microsoft Office Suite and electronic medical record systems. Demonstrated ability to use municipal case management tools and child protective service documentation. Strong typing, writing, and documentation skills for accurate record‑keeping. Physical Requirements Capable of walking, standing, and climbing stairs for up to 1 hour as necessary. Ability to lift up to 50 lb, carry up to 95 lb, and use a wheelchair as required.
