We are seeking a compassionate and motivated PRN Physical Therapist to join our growing Home Health team. In this role, you will help patients regain mobility, improve strength, manage pain, and restore independence — all from the comfort of their homes.

What You’ll Do

As a Physical Therapist, you will:

• Perform comprehensive patient evaluations and develop individualized treatment plans
• Assess functional limitations and establish appropriate therapy goals
• Deliver skilled therapeutic interventions to improve mobility, strength, balance, and overall function
• Monitor patient progress and modify treatment plans as needed to maximize outcomes
• Educate patients and caregivers on exercises, safety, and ongoing care strategies
• Maintain timely, accurate, and compliant clinical documentation
• Collaborate with physicians, nurses, case managers, and interdisciplinary care teams
• Follow infection control and universal precaution standards during patient care
• Support the agency’s mission of providing compassionate, high-quality home health services

Qualifications

• Graduate of an accredited Physical Therapy program
• Current and unrestricted Physical Therapist license
• CPR Certification required
• Minimum of 1 year of home health experience preferred
• HomeCare HomeBase experience preferred
• Current Driver’s License and reliable transportation
• Valid Auto Insurance

Physical Requirements

• Ability to stand, walk, bend, squat, kneel, and climb for extended periods
• Ability to lift, push, pull, or carry up to 100 lbs.
