Compassionate Hospice RN Case Manager

Gentiva Hospice is seeking a dedicated Registered Nurse (RN) Case Manager to lead in-home hospice care. You will perform comprehensive assessments, deliver skilled nursing interventions, and collaborate with an interdisciplinary team to develop and update compassionate care plans for patients with life-limiting illnesses. You will educate patients and families, manage medications and symptoms, and ensure accurate documentation while maintaining high standards of infection control and compliance. #J-18808-Ljbffr Gentiva
