Oakwood Rehab and Nursing Center SSA Job Duties Inventory all resident belongings, including admissions, discharges, room changes, and current residents. Document all inventoried items on the inventory sheet and in PCC. Conduct psychosocial groups and document resident participation. Supervise smoke breaks by monitoring, observing, de‑escalating, and redirecting behaviors as needed. Report any concerns to the appropriate supervisor. Ensure residents sign and complete the smoking contract. Conduct room searches for contraband items. Educate residents on the facility handbook and house rules and obtain required signatures. Schedule dental appointments through Senior Dental and report any refusals of care or related behaviors to the appropriate supervisor. Complete daily Wander Guard checks, documenting the date and time, and report any changes. Assist with room changes and ensure families are notified. Maintain the token store, including updating token cards, points, and any necessary changes. Ensure all mail and packages are distributed daily. Monitor the dining room to ensure meal trays are distributed during mealtimes. Communicate all changes in resident condition, functioning, and behavior to the appropriate supervisor.
