Title: Program Coordinator – Sweeney Place Workforce Housing
Regular Full time: 40 hrs./week…Non-exempt (occasional evenings and weekends)
Schedule: Monday – Friday; 9am – 5:30pm
Pay range: $33 - $35 per hour DOE
Reports To: Senior Director of Housing Programs
Start Date: Open until filled

We’re looking for a Program Coordinator to join our adult shelter and housing team and support residents as they build stability and self-sufficiency. In this role, you’ll work with participants in our newly opened 26-unit workforce housing program, which provides affordable housing for adults and families transitioning out of homelessness. Residents are employed in the local community, and you will help them maintain their housing, navigate challenges, and move toward greater independence. Through consistent, onsite support, you’ll play a key role in ensuring residents feel supported, seen, and genuinely set up for long-term success.

Education or Training Equivalent
- Bachelor’s degree in social work, social welfare, psychology, or a related field, or at least 3 years of experience in a related residential or housing program.
Qualifications and Abilities
- Minimum of three (3) years’ experience working with under-resourced or low-income populations; experience working with individuals experiencing homelessness preferred.
- Experience in residential and/or housing programs.
- Bilingual (English/Spanish) preferred.
- Knowledge of Housing First and harm-reduction principles.
- Working knowledge of SSI, SSDI, and SDI.
- Experience with data entry systems (HMIS, Salesforce, or similar).
- Proficiency with Microsoft Word, Excel, cloud-based systems, and Adobe.
- Comfort working in a culturally and ethnically diverse environment.
- Working knowledge of substance dependence disorders and mental health disorders.
- Demonstrate the ability to seek feedback, ask questions, and apply constructive suggestions.
- Ability to interact with residents in a compassionate, flexible, and respectful manner, maintaining professionalism in all interactions.
- Efficient in handling multiple task simultaneously and making quick, informed decisions in fast paced environments.
- Ability to work effectively both independently and as part of a collaborative team.
Physical Requirements
- Ability to sit and use a computer mouse, keyboard, and monitor for moderate periods of time (1 – 3 hours per day).
- Ability to stoop, crouch, kneel, or crawl occasionally.
- Ability to lift or move objects up to 40 pounds.
Key Responsibilities
- Provide housing-focused case management in one-on-one settings for up to 24 single adults and group settings.org
- Incorporate CalAIM care coordination and housing stabilization strategies into individualized housing and care plans, in collaboration with healthcare, behavioral health, and community-based partners.
- Identify and address barriers to housing stabilization, including chronic homelessness, mental and physical health needs, substance use disorders, under-employment, lack of income, and other systemic barriers.
- Skillfully prevent and/or resolve conflicts skillfully using a strengths-based approach.
- Manage a full caseload and bi-weekly/monthly meetings, with strong organization, time-management, and prioritization skills.
- Participate in weekly supervision, program meetings, all-staff meetings, and required training.
- Respond to urgent and crisis situations, including coordination with emergency medical teams, fire department, law enforcement, psychiatric services, and community partners as needed.
- Prevent and resolve conflicts using a strengths-based, trauma-informed approach.
- Provide supportive services, guidance, and referrals to community resources.
- Engage program participants regularly and consistently to support program participation and progress toward housing stability.
- Able to maintain professional boundaries and set limits.

Documentation and Compliance
- Complete timely, accurate, and compliant documentation, including billable case notes, assessments, care plans, and progress updates, in accordance with policies and funder requirements.
- Ensure client records clearly demonstrate eligibility, service delivery, engagement, and progress toward housing, health, and personal goals.
- Maintain accurate and timely data entry in HIPPO (Salesforce database) and HMIS.
- Meet all documentation and case management deadlines, including intake, enrollment, reassessments, housing goals, and discharge documentation.
- Participate in quality assurance, monitoring, and audit processes, including responding to documentation reviews and corrective actions.
Case Management, Housing-Focused Services, and Outcomes
- Develop, implement, and regularly update individualized, client-centered housing and care plans.
- Monitor, document, and adjust plans proactively to address changing needs or barriers.
- Coordinate services with internal teams and external partners to ensure continuity of care and timely referrals.
- Document housing and service outcomes to support program performance measurement and funder reporting.
- Develop, implement, and monitor financial literacy services tailored to covering topics such as budgeting, credit repair, savings strategies, and employment preparedness.
- Assist program participants in housing retention, job readiness skills, job placement and retention services, education on living skills, navigating resources and referrals, eviction diversion, and addressing physical and mental health issues.
Property Management and Housing Placement
- Assist the Senior Director of Housing with client identification and screening for housing eligibility, leasing, and client move-ins
- Assist the Senior Director with refining program policies and practices, unit setup and purchasing, and facility troubleshooting
- Promptly submit documentation to the Marin Housing Authority for the issuance of client vouchers and rental calculations/recalculations
- Assist with property management, including but not limited to creating and tracking maintenance requests, maintaining site presence across the housing campus, tracking client rental payments, issuing legal notices, and documenting tenancy challenges
- Proactively monitoring of building needs, rounds, coordinating with facilities
Written & Verbal Communication Skills:
- Ability to read and interpret documents, write routine reports and correspondence, and speak effectively before small groups of clients or employees of the organization.
- Communicate clearly and professionally with supervisors, team members, volunteers, and program participants.
Relationships:
- Foster open and collaborative relationships with colleagues, working as a cohesive team with all Homeward Bound staff.
- Uphold strict confidentiality regarding all program participants.
