Home Health Registered Nurse (RN)Seeking a dedicated and compassionate Home Health Registered Nurse (RN) to provide skilled nursing care to patients in their homes throughout the assigned North Carolina service area. Schedule & Work Details Coverage Areas: Robeson, Hoke, and Cumberland Counties Schedule: Full-time Shift: Monday through Friday, 8:00 AM – 4:30 PM Weekly Hour Guarantee: 40 hours On-Call Requirement: Yes Weekend Requirement: Yes Mileage Reimbursement: Yes Pay Rate: $51.00 - $53.00 per hour Key Responsibilities Provide skilled nursing services and comprehensive patient assessments in the home while developing and implementing individualized care plans. Complete accurate clinical documentation and OASIS assessments while maintaining timely and compliant records. Provide patient and caregiver education, medication management, wound care, and other skilled nursing interventions as appropriate. Coordinate care with physicians and interdisciplinary team members, monitor patient progress, and support continuity of care and positive patient outcomes. Required Qualifications Active, unrestricted Registered Nurse (RN) license. Strong clinical assessment, communication, and organizational skills. Ability to work independently in a field-based setting. Availability to participate in required on-call and weekend coverage. Preferred Qualifications Previous home health nursing experience. Experience using Homecare Homebase (HCHB). Experience managing a home health patient caseload.
