Registered Nurse (RN) - Case ManagerWe are seeking a dedicated Registered Nurse (RN) specializing in Case Management to join our healthcare team. This position requires an immediate start and offers a rewarding opportunity for those with acute care experience.Key ResponsibilitiesProvide comprehensive case management and discharge planning for patients.Maintain a patient-to-nurse ratio of 1:20 during the day shift.Collaborate with multidisciplinary teams to ensure optimal patient outcomes.Adhere to best practices and standards in case management.Participate in weekend and holiday shifts as required (every other weekend and available for holidays).QualificationsMinimum of 2 years of experience in acute care case management or discharge planning.Current California state nursing license required; pending licenses are accepted but must be active before the start date.Must be a first-time traveler and agree to undergo screening as necessary.Availability to work with no more than 7 days of restrictions on Time Off (RTO).Facility HighlightsType of Facility: Acute Care FacilityDress Code: Business Attire or ScrubsShiftsThis position requires day shifts, working 5 days a week with 8-hour shifts from 08:00 to 16:30.If you have any questions about this opportunity, please reach out for clarification.Client Details State CA
