PT - Physical Therapist

The Outpatient Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans in accordance with the patients' needs, goals, and physician direction. This role is responsible for the delegation and direction of all discipline-specific therapy services for assigned patients and ensures the design of functional treatment programs for effective community reintegration. Responsibilities Conduct comprehensive evaluations of assigned patients. Utilize clinical reasoning to develop treatment plans and establish goals focused on discharging patients within an established length of stay. Maintain clear, concise, accurate, and timely medical record documentation in compliance with state and federal guidelines. Participate in the student program by providing a positive learning environment with supervision and evaluations. Create and actively pursue an individualized professional development plan. Adhere to the discipline-specific State Practice Act and maintain professional licensure in good standing. Understand performance improvement methodologies and contribute to the organization's performance initiatives. Required Experience / Certifications / Licensure Degree in Physical Therapy from an Accredited School. Minimum of 1 year of outpatient clinical experience. Current CPR Certification.
