This is a fully remote position, allowing you to provide care from the comfort of your own space. Despite the virtual aspect, collaboration and communication with a multidisciplinary team of healthcare professionals, including physicians and nurses at the hospice center, are integral to your success. Our team emphasizes flexibility to meet client needs, which may include evening and weekend hours. You will use telehealth platforms and other technology to connect with clients and facilitate sessions.

Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to evaluate clients' mental health needs and to identify appropriate interventions.

• Provide therapeutic interventions, tailoring individual, family, and group therapy sessions based on evidence-based treatment modalities, such as cognitive-behavioral therapy and trauma-informed care.

• Develop and implement individualized treatment plans that address clients’ specific goals and therapeutic outcomes.

• Collaborate closely with the healthcare team to coordinate care and integrate mental health services with physical health support, ensuring a holistic approach to client well-being.

• Offer crisis intervention services, including risk assessments and safety planning, as emergencies arise.

• Maintain robust documentation in accordance with ethical and legal standards, ensuring timely and accurate reporting of client progress.

• Facilitate group therapy sessions, psychoeducational classes, and support groups to promote community and shared healing among clients.

Patient Population / Client Focus\n

In the hospice setting, you will primarily work with adults and elderly patients suffering from terminal illnesses. This vulnerable population often faces significant emotional distress, including grief, anxiety, and existential questions about death and dying. Your role will involve guiding clients through these profound challenges, ensuring they feel heard and supported through their mental health journey.

Qualifications\n

• A Master's degree in Social Work from an accredited institution.
