Case Manager

Southwest Behavioral and Health Services is seeking a dedicated and empathetic Case Manager who can help us fulfill our mission. We are seeking someone who enjoys coordinating with people, ability solve problems, organized and excellent communication skills. Does this describe you? Job Preview at a Glance:
- Intake screenings (Completing intake paperwork and referrals)
- Completing annual behavioral health assessments, including service planning and support planning
- Facilitating CFTs and ARTs
- Facilitating groups
- Identifying gaps in care for our members
- Completing SMI and SED evaluation
- Minimum of 21-year of age
- Bachelor's degree in the behavioral health field plus one year of relevant experience in behavioral health
- A combination of experience and education totaling 2 years is acceptable.
- Valid AZ driver's license & motor vehicle clearance.
- Must have fingerprint clearance issued by the Arizona Department of Public Safety or be eligible to receive upon hire.
