Clinical Pharmacotherapy Specialist - Adult Transplant Services

Position Clinical Pharmacotherapy Specialist - Adult Transplant Services. The Clinical Pharmacotherapy Specialist will provide safe, efficacious and cost-effective pharmacotherapy to patients in their designated dispensing area. Responsibilities Fulfills the core values and mission of NYU Langone Medical Center and incorporates them into the performance of duties. Maintains professional competency and certification. Identifies and provides expertise for Medication Use Evaluations. Responsible for Pharmacy Resident precepting; Pharmacy student mentoring; and scholarly activity. Participates in hospital committees as identified and assigned by the Director of Clinical Pharmacy Services. Documents and reports clinical interventions, adverse drug reactions, and medication errors. Develops and implements clinical practice guidelines to enhance pharmaceutical care and medication safety. Evaluates and reports measures of patient encounter productivity and identifies methods to improve efficiency. Identifies and develops policies and procedures that optimize pharmacy operations while meeting accreditation standards and regulations. Contributes to the professional development of staff, including pharmacists and clinical pharmacists. Oversees quality assurance of clinical pharmacy programs to improve quality of care. Develops and maintains a high-level clinical pharmacy practice. Participates in multidisciplinary bedside rounds. Provides pharmacotherapeutic, pharmacokinetic, and pharmacoeconomic consultation. Participates in medical response teams and cares for patients through transitions of care with medication counseling and education. Minimum Qualifications Doctor of Pharmacy degree from an ACPE accredited school of pharmacy. New York State Pharmacist Licensure. PGY1 pharmacy residency training. PGY2 pharmacy residency training or equivalent professorship at a school of pharmacy. Board certification in Pharmacotherapy or other specialty required within 18 months of hire. Self-motivation, good communication, organizational, and interpersonal skills. Licenses Pharmacist License – NYS.
