Registered Nurse - Home Health

Job Summary:
Seeking an experienced Home Health Registered Nurse to manage high-volume patient caseloads in a dynamic healthcare environment. This role requires close adherence to clinical protocols and regulatory standards to provide exceptional patient care.

Responsibilities:

• Coordinate and deliver comprehensive home health nursing care to 5-6 patients daily.
• Perform accurate and thorough assessments, updating the OASIS documentation to ensure compliance.
• Administer treatments, medications, and therapies in accordance with physician orders and regulatory guidelines.
• Educate patients and families on disease management, medication adherence, and wellness strategies.
• Collaborate with interdisciplinary teams to develop and modify individualized care plans.
• Maintain detailed, timely documentation of all patient interactions and clinical activities.

Qualifications:

• Valid CA RN license, active and in good standing, with no pending or temporary status.
• Minimum of 2 years’ experience in home health, including at least 1 year of travel experience.
• Recent OASIS expertise, with the ability to complete the OASIS E11 form accurately.
• BLS certification required.
• Ability to work the scheduled shift of 8:00 AM to 4:30 PM, including weekends as specified.
• Strong clinical assessment skills and knowledge of home health regulations.
