Social Worker Position
This Social Worker Position serves as a Behavioral Health Interdisciplinary Program (BHIP) Case Manager, independently providing expert clinical and case management services to Veterans served by the main campus Behavioral Health Interdisciplinary Program (BHIP). BHIP provides treatment coordination and service delivery for Veterans who require higher level mental health specialty care than can be provided in the primary care setting. BHIP teams are interprofessional and rely on the collective expertise of multiple disciplines to provide care for Veterans who have severe and/or complex mental health needs.
Duties include but are not limited to:
- Utilizes the Collaborative Chronic Care Model (CCM) to facilitate coordinated, proactive, population-based care.
- Adheres to referral management procedures focusing on timely and person-centered care coordination, as well as continuity for Veterans.
- Utilizes procedures to promote re-engagement, referral, or transition to other levels of care, as clinically determined based on patient's stability and recovery.
- Conducts psychosocial assessments.
- Develops treatment plans in collaboration with the Veteran/family and with the interdisciplinary treatment team.
- Independently provides consultation services to other staff regarding the psychosocial needs of Veterans and the impact of psychosocial problems on health care and adherence to treatment plan.
- Has working knowledge and experience in use of medical and mental health diagnoses, disabilities and treatment procedures including acute, chronic and traumatic illnesses/injuries, common medications and their effects/side effects, and medical terminology.
- Establishes and maintains effective therapeutic relationships with Veterans and their families.
- Works with patients and families who are experiencing a variety of psychiatric, medical and social problems utilizing individual, and family counseling and therapy skills.
Work Schedule: Monday-Friday 8:00am - 4:30pm; subject to change based on the needs of the facility

Pay: Competitive salary and regular salary increases.
Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year).
Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child.
Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66
Retirement: Traditional federal pension (5 years vesting) and federal 401K with up to 5% in contributions by VA
Insurance: Federal health/vision/dental/term life/long-term care (many federal insurance programs can be carried into retirement)
