Nurse Practitioner or Physician Assistant - PRN

Role Overview This position involves conducting comprehensive in-home health assessments for adult and geriatric patients, reviewing medical history, medications, social factors, and health risks to improve care quality and identify unrecognized conditions. Providers collaborate with physicians, case managers, and family members, administer tests, and document findings per protocols. Services are delivered at members' residences, including homes and Skilled Nursing Facilities. Key Responsibilities Perform assessments, health education, and referrals Administer point-of-care and lab tests Coordinate with healthcare team and follow up on care plans Document visits accurately and thoroughly Qualifications & Skills Master’s Degree or equivalent with NP licensure and prescriptive authority in the state Board certification in relevant specialty (Adult, Gerontology, Family, Emergency, or Acute Care) Current BLS, ACLS, or CPR certification Strong communication, computer skills, and a valid driver’s license with insurance coverage Preferred: 1+ year NP experience; new grads encouraged to apply Additional Information Offers flexible scheduling, competitive pay based on assessments completed, mileage reimbursement, and support for continuing education and licensure.
