This role is responsible for managing refill requests, resolving prescription issues, and ensuring timely follow-up with providers and pharmacies. This position works in coordination with front office staff to support scheduling, patient communication, and overall clinic flow, while ensuring documentation is accurate and completed on time.If you are organized, detail-oriented, and able to follow through in a fast-paced environment, we encourage you to apply and become part of a supportive, caring team dedicated to quality patient care.As an Outpatient Care Coordinator, your responsibilities include:Welcome and assist clients in a professional and friendly mannerWhen appropriately credentialed, may perform vital signs as directedAssist clients with telehealth appointmentsEnsures all medication-related calls are returned within one business dayEnsures clients receive medications by managing refill requests, resolving prescription issues, and coordinating with providers and pharmacies, without making clinical decisionsCoordinates client services in collaboration with outpatient psychiatric providers and staffCommunicates with clients regarding appointments, medications, and follow-up needsWorks with scheduling staff to arrange appointmentsEnter and maintain accurate information in the EHRComplete required forms and documentationUses appropriate forms, both paper and electronicComplete FASAMS admissions and quarterly updatesEnsures documentation is accurate, complete, and timelyPerform other duties as assignedRequirementsPREFERRED:Experience with scheduling, referrals, insurance authorizations, or care coordination preferredBilingual skills are a plusAssociate's or Bachelor's degree preferredREQUIREMENTS:High school diploma or equivalent required1–3 years of experience in a healthcare, behavioral health, or outpatient settingProficiency in Microsoft Office (Outlook, Word, Excel)Must pass Level 2 Background through DCF and AHCA Clearinghouse.
