Job Description
Title: Social Services Practitioner - Social Work

Schedule:
- Day Shift: 8:00 AM - 4:30 PM
- Monday to Friday with occasional weekend shifts.

Assignment Details:
- Contract Length: 13 weeks
- Guaranteed Hours: Not specified

Requirements:
- Master's Degree in Social Work or equivalent combination of education and experience: Required
- Minimum of 1 year of experience: Required
- Preferred three years experience in a social agency, hospital, or other organization dealing with health and/or welfare
- Licensed Clinical Social Worker (LCSW): Preferred
- Crisis Prevention Intervention (CPI) or Techniques for Effective Aggression Management (TEAM) or TEAM-A: Required

Roles & Responsibilities:
- Assess client needs and develop appropriate care plans.
- Provide counseling and support to clients and families.
- Collaborate with multidisciplinary teams to ensure comprehensive care.
- Maintain accurate and timely documentation of client progress.
- Advocate for clients' access to necessary resources and services.

Compliance / Notes:
- CPI training can be provided on-site after hire if needed.
