Your next nursing opportunity awaits! Seeking a dedicated Registered Nurse with proven OASIS expertise for a 13-week full-time home health assignment. This role offers the chance to deliver meaningful care in a patients home environment, supported by a professional and responsive clinical team.

Schedule:

• Full-time, Monday through Friday
• 13-week contract, with potential to extend
• On-call participation may be required
• Structured daily patient visit expectations

Essential Responsibilities:

• Deliver skilled in-home nursing care and interventions for diverse populations
• Complete accurate patient assessments and promptly submit OASIS documentation
• Work collaboratively with physicians and interdisciplinary care teams to tailor care plans
• Provide wound care, medication management, and patient/caregiver education
• Monitor and report changes in patient conditions to the clinical team in a timely manner

Preferred Qualifications:

• Active RN license in good standing
• Minimum of one year of recent home health RN experience
• Demonstrated proficiency with OASIS documentation (required)
• Strong familiarity with HomeCare HomeBase (highly preferred)
• Excellent organizational and time management skills to handle a consistent patient load
• Reliable transportation and valid drivers license
