Housing Stability Case Manager

Hope Solutions

Hope Solutions is seeking a Case Manager to deliver home-based case management and ongoing assessment for housing stability in Contra Costa County. You will link clients to medical, mental health, recovery, financial, and legal services and support landlord/tenant relations to prevent eviction. The role requires a trauma-informed, strength-based approach, collaboration with Housing Authority and Health Housing staff, and mobile, community-based service delivery. #J-18808-Ljbffr
