Perform initial and ongoing physical therapy evaluations; develop and adjust a Plan of Care with short‑ and long‑term goals, in collaboration with other staff as needed. Document patient progress according to approved medical and facility documentation procedures; develop discharge plans with home exercise programs and caregiver instructions. Evaluate mobility, strength, and current function; implement the Plan of Care on a consistent schedule and modify as patient progress dictates. Maintain open communication with therapy staff, department colleagues, and referral sources; supervise physical therapy assistants as appropriate. Maintain patient records and ensure compliance with certifications, authorizations, and third‑party payer requirements; pursue ongoing professional development through workshops and literature.
