Responsibilities: Coordinate and manage the assessment and care planning process for residents, including the development and implementation of Individualized Care Plans Complete and submit accurate and timely MDS assessments, Care Plans, and other required documentation Collaborate with interdisciplinary team members to identify and meet the unique needs of each resident Provide education and support to residents, families, and staff on the MDS process and care planning Stay up-to-date with regulatory requirements and industry best practices to ensure compliance Requirements: Active Licensed Practical Nurse (LPN) or Registered Nurse (RN) in the state of Missouri Strong understanding of long-term care services and regulations Excellent communication, organizational, and problem-solving skills Ability to work effectively in a fast-paced environment and prioritize multiple tasks Strong analytical and critical thinking skills Ability to maintain confidentiality and discretion when handling sensitive information Benefits: Advanced Pay (hourly employees) Competitive Wages Advancement Opportunities Exclusive Aspire Senior Living Perks #J-18808-Ljbffr
