Position Overview Occupational Therapist – Full Time Sun West Choice Healthcare & Rehabilitation in Sun City West, AZ is seeking a full‑time occupational therapist to join our in‑house therapy team. This position is open to OTs at all experience levels. Responsibilities Screen and evaluate patients with cognitive and/or ADL physical functioning disorders and develop appropriate plans of care following regulatory and clinical practice standards. Utilize standardized assessments and evidence‑based practice to support clinical interventions. Provide rehabilitative, skilled and medically necessary treatment interventions to patients per regulatory and clinical practice requirements. Administer procedures such as ADL retraining, therapeutic activities, therapeutic exercises, modalities, use of supportive and assistive devices, etc. Assess patient’s progress and adjust treatments accordingly in the clinical record in accordance with regulatory and clinical practice requirements. Evaluate treatment outcomes for effectiveness; modify services in relation to patient evaluation and refer as appropriate. Provide consultation and counseling to patients, families, caregivers and other service providers regarding physical disorders. Effectively communicate with supervisor and other health team members regarding patient progress, barriers and treatment plans. Provide comprehensive discharge summary of services per regulatory and clinical requirements.
