Community Health Navigator

MedZed seeks a Community Health Navigator (CHN) for the Complete Connect Program in New Jersey. This full-time, field-based role focuses on finding, enrolling, and supporting Medicaid members throughout a six‑month enrollment, with strong emphasis on outreach and follow-up. You will manage a caseload of 45–55 members, conduct in-person visits, coordinate social determinants of health, and document progress to close care gaps. #J-18808-Ljbffr
