St. Croix Hospice in Columbia Township is seeking a Registered Nurse Case Manager dedicated to providing compassionate care. In this full-time role, you will be the cornerstone of patient support, developing individual care plans and ensuring exceptional symptom management. The ideal candidate will have a nursing degree and at least one year of acute care experience, ideally in hospice or home healthcare.
