Responsibilities

• Conduct comprehensive physical therapy evaluations for residents and develop individualized treatment plans targeting mobility, strength, balance, and functional independence.
• Implement and adjust therapeutic interventions including therapeutic exercises, gait training, manual techniques, and transfer training as appropriate.
• Monitor and document patient progress, update treatment plans, and contribute to discharge planning in collaboration with the interdisciplinary team.
• Perform timely, accurate documentation of assessments, goals, progress notes, and outcomes in accordance with facility policies and regulatory standards.
• Collaborate with physicians, nurses, therapists, and other team members; participate in care conferences and family communications as needed.
• Ensure safety and proper use of equipment, maintain infection control practices, and adhere to all facility policies and ethical standards.

Requirements

• Active physical therapy license in the United States per state requirements.
• Degree in physical therapy from an accredited program (DPT or MPT).
• Ability to work in a skilled nursing facility setting with older adult residents.
• Strong documentation, communication, and teamwork skills.
• Flexible and able to participate in a travel assignment for a 14-week contract with the specified schedule.
